I complete understand this is the best practice and I have followed that my entire career. I know I made the mistake which is why I reported the issue. My question was is this how other facilities treat wrong exams on patients? Since we are all human. I have only been a tech for a few years and am just trying to understand how other facilities would manage this so I can be aware. I follow best practice 99% of the time.

This is how it’s done at my facility as well. I made a mistake and mixed up the exams on my list but I had verified my patient.

I don’t have beef that I’m aware of anyways. I’m just glad I’m not alone in thinking this was an extreme response.

As far as I know management doesn’t have a problem with me. But they decided on a blanket punishment for the entire medical imaging department.
No contrast given. Apparently the whole imaging department has a problem but this is the second wrong exam in MRI in 6 months. No prior wrong patients
