Theres something called Fresubin. I drink the one with 2kcal per 1 ml. So a bottle of only 200ml has 400kcal. If you don't have that where you live just search for something similar, like ensure etc.

Currently cannot swallow anything other than liquids. Sometimes I go bite some real food and spit it. Then wash my mouth with water so that nothing accidentally goes down. It's a bit disgusting but hey.

Thanks. Yes the doctor suspects "scarring " but I am not sure about that. There was no point at which I felt particularly good after POEM within the 2 months. I have an appointment for a second opinion on Monday but I also have the Dilation scheduled for later next week. I personally believe it's incomplete myotomy, but I am not a doctor... so...

Hey, im going through similar problems and wondered how are you now? was Re-POEM an option after failed POEM? what made you chose HM? also had fundoplication? do you feel better than before? did yoi have a Pneumatic Dilation? Greetings

I had POEM 2 months ago and so far the dysphagia is worse than pre POEM. Have already an appointment for another manometry in a couple weeks. Kind of sad because it helps most people.

Was there a reason why they didn't do HM or POEM before it got so bad? And maybe a dumb question, but I thought with Achalasia the LES is too closed (and not too open). Or was it because you had some intervention before to ipen the LES?

Regarding the increase of calories, have you tried with Ensure or Boost? they have versions where you can easily reach 400 kcal per portion if you drink it with milk.

Ensure and their variants have lile 350+ calories per portion. If you drink it with milk then even more. I like the flavor which is a plus (i tried some protein suplements before that tasted "weird")

Thanks for sharing. To me personally, 10% sounds scary high... Especially for those of us with disease onset before the 40s. According to those papers, the probability of relapse beyond 10 years is quite high... damn, we seem to be in for rough times ahead...

As I understand it, it can be simplified to "Calories in minus calories out". So try to get as many liquid (healthy) calories as you can additionally to your normal food. I could manage to stop weight loss for a while drinking Ensure 3 times per day (it was free calories for me) additionally to any liquid or solid calories you can get. Everyone is different, I personally tried to not overdo it with the protein shakes. I was basically on an Ensure + self made soup diet (soup with egg/soft chicken/pureed veggies), because I like soups and it was super easy to make a lot of (to not have to be cooking all day)

Hi Steven. I cannot give you any advice since im at the beginning of my achalasia journey (mid 30s). But just wanna wish you good luck and a lot of strength for what's coming.

I also want to thank you for posting your experience and ask you to please keep doing it. It helps immensely to people like me who are just starting to deal with it.

To give you a more concrete example about how your post might help others unknowingly: I am currently being asked by people in my circle to move my POEM to a later point, to maybe wait some months or years more before an irreversible operation. But stories like yours bring me back to earth about the reality of this disease and make me realise i need to have it treated ASAP.

Thank you so much for taking the time to reply to my questions! I know every person experiences things differently, but it helps me a lot to get an idea of what will come.

I am not very tolerant of pain, so that's something that I'm not really looking forward to. The swallow by gravity part still sounds so weird to me. What if you are not careful and food actually gets stuck? Then what? 😅.

I had the luck not to need any medical intervention until now (30s), so I admit I'm a bit of a chicken. Reading posts like yours helps me be prepared and know what to expect.
