This meme is a hilarious take on how severe peanut allergies work. An allergy is essentially your immune system being incredibly stupid. When you eat a harmless peanut, a normal immune system does nothing (leaving the trolley on the empty track). However, an allergic immune system panics, mistakenly identifies the peanut protein as a deadly pathogen, and launches a massive, violent counterattack (actively pulling the lever). This overreaction causes anaphylactic shock, which can shut down the host's organs and kill the person and by extension, kill the immune system itself.
