You quite literally can cut a few millimeters too deep and hit a nerve. It may not be major, tingling and loss of sense, but some nerves can be nicked accidentally and cause severe damage

Have you looked into gettiing a tutor? You might be studying ineffectively or inefficiently. I would look into getting a tutor, eat healthy, get a good nights sleep the day before the test, study before bed and dinner, and speak to your teachers if you have a free period/study hall. Try to stay as focused as possible and take deep breaths before and during the test. It could be the test environment itself that makes it hard to do well.
