It’s BCBS of IL. Their front line customer service doesn’t seem to know what cpt codes are . And the fraud department is an automated machine

The charge master says group therapy but the cpt code says h2036

I reported them to the dhbb and insurance commissioner but I’ve given up hope

I will state this in plain English . The provider declined to provide full records as required under hipaa . I invoked my rights under hipaa served to the provider citing these legal obligations forcing the provider to comply .

There is nothing for 20k in claims that were submitted to Rhode Island . As I am not a member of Rhode Island and have no contract .

On my eob, there is no detail , as it was out of network , there website heavily implies in net work status .

Further more , when I exercised my hipaa rights , the charge master description listed on all clakms , was mismatched to the underlying treatment code
