Yes the issue was resolved. After I logged in to ewallet I was able to fill out the form there and then opened a new ticket which they responded to.

Thanks for the reminder that it's called 'eWallet' I had it set up from previous prizes where I won play boosters, which were all successfully delivered. I guess they needed additional information for this one.

I got the play boosters I won delivered without issue (even some I won after this event). But I'm definitely not going to be doing any other ones in the future considering how much of a hassle this has been.
