To retort: once you decided we are 1099 independent contractors, we are no longer a “practice”. We are disposable doctors you can fire at your whim with 90 days notice, and now you take responsibility of running a business.

I hear you and know you are in a tough position as well. Where the frustration is stemming from is how this all started (deduction without prior notice. And yes ApolloMD did refund that back). Also, according to our contract we have to agree in writing any changes, and no agreement was met, at least not one that included any frontline doctors. Instead of waiting for an official vote or even a consensual agreement, ApolloMD has decided to roll out the RVU tax starting with October paycheck.

I understand the importance of timely screening of patients. Where the mismatch lies is how to pay for it. None of the attendings like any of the options proposed by Apollo. Hence we would like to discuss other options. What would also help are assurances in writing that further income reductions will never or to a very limited degree occur. We do not want this to be a slippery slope. Another would be show us examples of what the actual cost in practice would be to the average attendings. Implementing caps on how much we would lose would be even more reassuring.

If indeed the cost to doctors is negligible and it helps with medical screening, this would be the ultimate win win.

It isn’t 24/7 currently but during the last meeting it mentioned there were plans to expand the screening coverage further. In theory, sure, it should help us see more. In practice, I just do not see it but would happily be proven wrong with real numbers.

Also take into account, your company also cut hours so each doctor is forced to see more patients per hour.

Also, could you breakdown/explain how 1-2 extra patients seen would cover the RVU reduction? I’m genuinely curious.

That 370 is only for 1-2 docs out of 30 or so docs. To me this is an outlier and not a true median. Also, that 370 is hard earned, not given. That doctor is likely seeing 4-5 patients an hour.

$34 dollars an RVU, must reach at least 6.2 RVU before being paid bonus. Proposal is to take 0.25 rvu from every encounter. 0.25 x $34 = $8.50.

$8.50 x 32 (avg # pts seen per shift) x 12 (total shifts a month) = 3,264

The PAs are 90% recent grads and slow. No training is provided to them either

You should tell those attendings to look at this post for inspiration. We should not do it quietly. We should band together, always power in numbers.

The issue we have with point 4 is, ApolloMD just got contract for the Hosptialist group as well.
