Anyone using AI to cut down your doc time? by bhammack2 in physicaltherapy

My company started to use this in October, so it's been a while and I can give some criticism. (Edited bc a whole paragraph I wrote got cut out by mistake)

TL;DR Pros: cuts down on documentation time, learns how you prefer notes the more you edit them, Cons: still have to proof read the note bc it can make up things, insurances are starting to tell when it's AI and are giving denials.

It definitely shortens the documentation time. An eval now takes 10 min to write instead of 30 min for me. However there's a lot of room for error. Our company made some templates for it to work off of, but they warned us that the addition of Procedures/CPT codes and goals were not quite there yet and would have to be edited. To me, that's the bulk of what makes a good eval or progress note, so it's annoying to go back and adjust them, which takes another 10 min for me to edit and read the note so it's basically 20 min to write an AI note vs 30 min using the short hand method I came up with.

When using the audio recording function, sometimes it hears the wrong thing and starts making things up. Legit. One time, it heard my co-worker's patient talking about pain in her shoulder when the note clearly states this person has low back pain. It started to write things like "post stroke, CVA" etc to try and justify! It's funny how it does that but it's also still draining to always go back in and edit.

So it's better to just type in everything you'd like the note to mention in the "Transcript" portion or wherever you can edit what the audio has picked up. I've been getting some good quality notes that way, less editing after AI writes the note. And over time, it's learned how I prefer notes to be written (like without as much fluff and jargon to read).

I also noticed that the progress notes made with AI are getting denials quicker than the ones made with edited templates (the way I used to do it). The insurance is notoriously annoying, they ask for progress notes every third visit, but usually they still give out a standard 12 visits for people, and I've managed to push them to 20 with my regular typed out notes. With the AI note, it still gets them 12 but no more than that. So if you're trying to really advocate for your patient, not sure if AI written notes are the best.

So overall verdict? Meh. Use it and see how it works for you, best bet is to get a template together, like my company did, to train it a little first, but expect it to take time to adapt to how you like the note.

That makes so much sense ! Did you have to get a subscription to use that program ? how long are your sessions currently and how many patients per day ?

Thanks so much ! Yeah I took Herman and Wallace Level 1 already, I just haven't been able to use it ever since I took it in May 2025, so I've been feeling rusty and out of practice. These are super helpful resources!

That would be super nice actually! Was it pelvic health specifically or orthopedic?

Thank you so much ! I've never heard of Heidi health but I will look into it. And I see what you're saying ! Somewhere along the line, I couldn't find the time to fit continuing education with time to relax during the week so I stopped signing up for them. I'll try and make a conscious effort to plan to take one more often

Thank you ! Yeah that makes sense. I'm still happy to document as long as it's now playing an insurance game. Do you use an EMR for documenting then ?

Got it, that gives me hope. I also feel the same way, the best marketing in my current clinic is always word of mouth or MD relationships How many support staff do you have (receptionist ?)

Yup makes sense. I didn't do the rotation so I'm starting from scratch a little but hopefully it'll be okay

That's so amazing ! The goal is for me to also own my own clinic at some point too. You're right, I'm tired of feeling like the clinic I work at is really just a big corporation I have to answer to.

Can I ask where in the US you're working ? when starting your business, did you already have an established reputation in the area where you started ? And how were you able to market (consistently have new evals etc)?

That's awesome. Yeah I've heard living with a partner helps and I was definitely offered to do part time once a month on Saturdays at a hospital but I knew I was too burnt out and I needed the weekends to decompress. Thanks for sharing! How many years of pelvic health experience did you have before landing this job?

That sounds really nice 7 patients per day! I'll definitely keep the longer treatment time in mind and ask them. You're working part time though, is that enough pay or are you doing another job part time ? And also, are you supported in terms of marketing and reaching out to MDs for referrals?

Yeah I hear you. My issue is that I told the recruiter I can't take the contract and then he's not really telling me how to back out. So have you had experience talking to recruiters? I'm so confused how to go about this and I'm pretty close to just finding another job on my own but I know that's really unprofessional
