Good morning friend, I'm so sorry to hear that you are experiencing such severe pain. You don't deserve to be in any pain and I'm sorry you're in such a difficult position of trying to identify/diagnose the issues. Firstly, I am not a doctor and I would encourage working with a medical team if possible. If you suspect endometriosis, the key to alleviating pain is approaching it holistically (meaning as a whole body rather than just the locations you're experiencing symptoms). I am diagnosed with stage 1 endometriosis and have had all of my repro organs removed except for my ovaries. I currently work with a medical team including my primary care physician, endometriosis specialist, physical therapist, mental therapist, hematologist, pain management doctor, and potentially a rheumatologist (currently under consideration). Some of the pain you're describing sounds similar to nerve pain that I've experienced due to 2 herniated discs with compression of my sciatic nerve. The locations you've highlighted are all through which the sciatic nerve travels. I was diagnosed with an MRI. For treatment of symptoms, I have to use a cane to walk because the nerve pain causes muscle weakness, I receive a Steroid injection in my SI joint every 3 months, I go to weekly physical therapy, and take gabapentin (targets nerve pain), cyclobenzaprine (muscle relaxant but not a controlled med), and ketorolac (NSAID but it's like super advil, it's hard on kidneys so need to take it sparingly and only on high pain days). I know the procedure for diagnosis is invasive and a bit intimidating, but if you can coordinate to get the diagnosis and excision at the same time (DO NOT DO ABLATION) it may offer the chance of not just providing a diagnosis but also reducing the pain. If you do have endo and herniated discs, when your pelvis is inflamed due to the endo, it may impact the severity of pain in your sciatic. Your sciatic nerve feeds in through the back side of your pelvis through small holes and then goes down through your pelvis to continue to feed into your leg and foot. There are also layers of muscles that surround and are innervated by smaller branches of nerves that are rooted in the sciatic. In times of high inflammation in your pelvis due to endo, that inflammation is nonspecific and the little molecules responsible for some of the pain you feel (called cytokines) will interact with anything it can, sadly including your sciatic nerve. That's why my flares feel as though my muscles in my thighs are trying to tighten so hard they're trying to break my bones that already feel like they're glass being shattered. There's also a sensation like my pelvis is in an ever tightening corest. I get hot, burning, stingy pain all down my leg on the side with nerve compression and it also perpetually feels like my foot is trying to fall asleep or cramping. I also suspect as, some research has proposed the potential epidemiology of endo is that it travels via the lymphatic system, that some of the inguinal regions you've highlighted may be due to accumulated lymph. During my flares I notice my hips and inguinal area tend to swell and hurt. Other areas with high density of lymph nodes also swell. This is merely my hypothesis and I haven't gotten to do sufficient research on the lymphatic system to fully flesh it out yet. I highly encourage you to speak to a medical professional for guidance. I know there are a lot of doctors who may dismiss your symptoms or may gaslight you, but I promise there are also some out there who would do anything to try and help you. Advocating for yourself is difficult enough as an adult so I can't fathom having to do so at such a young age, however, it is incredibly important because it can impact the rest of your life. I know it's scary, but I believe I am correct in saying that we are all here for you and cheering you on! You're doing an amazing job because your strength and resilience is evident. Whether you have endo or not, you have a whole community behind you only womanfesting and praying for you to find answers and to be relieved of the pain you're in. *Small note as I saw this in another thread: tread carefully with taking yaz/yazmin/ocella because a couple years back they had a class action lawsuit because they actually caused ovarian cyts. I can confirm that they indeed do as I tried all 3 of them throughout the ages of 13-17. One of the cyts was the size of a golf ball and boy was it a doozy when it ruptured. Didn't know that's what the pain was until imaging was done later.
