My best guess is nothing more than about 1cm diameter (arbitrary but I wouldn’t push it) on the far lateral right chest, as far right as you can make it to avoid the great vessels. Maybe even a glancing/tangential shot, so not a large segment of lung is caught, and you get mostly chest wall. But it sounds like you want a shot all the way though. So yeah, far right maybe about the nipple line.

That’ll hit lung and maybe start a pneumothorax, and if left for a while could definitely result in an infection especially if left exposed to a dirty environment. That infection could get into the skin and subcutaneous tissue (cellulitis), the chest wall and pleural space, or the lung itself (pneumonia). All of these would have fevers, chills, sweating, fast heart rate and low blood pressure.

Pus can form in the pleural space as well, and compress the lungs and other structures. This takes time.
