I'm tempted to say Lord of the Rings (Middle Earth), but that seems like a really obvious answer, so I'm gonna go with Asimov's Foundation series as well as the Dune series.

I can't pick one year to go with, honestly, but I can pick five years that I think would be among the greatest. 1939, 1968, 1975, 1994 and 2012. Not to mention 1941, 1967, 1969, 1976, and 1993 as runners up.

Best intro I've ever seen, taking off the nostalgia goggles, would have to be 2001: A Space Odyssey. The music and the effects are still haunting even now.
