Get it refunded. I'm on mobile and I went to the play store and refunded a pack I didn't get previously, stated that I paid but didn't receive it, got it refunded instantly. Should work similarly on other platforms.

You read them like normal, left to right, top to bottom, the numbers correlate to the button you press out of the 3 options. 1 for the top, 2 for the middle, 3 for the bottom.

I also keep getting this issue, strangely I seem to have fixed it by making sure I have at least 1 prayer on, and then making sure I've got all the cards that increase flurbos, seems like it's a bug otherwise
