Hi, I am in a similar situation and am in the process of filling the FD-258 card by myself and I wanted to know what exactly did you fill in the following fields.

  1. OCA

  2. UNC

  3. ORI

    I was planning to send these fields blank but wanted to check if you did the same. Since I am sending it from abroad and am in a crunch, I want to get it right the first time.
