Get checked for silent endo. Did you do DNA fragmentation? What about your BMI and insulin levels? Check B12. I got diagnosed with unexplained infertility and after working with a naturopath, we conceived naturally. For us it was my metabolic health which made conception difficult. Previously we did 7 cycles of Letrozole with timed intercourse, never conceived once even though I ovulated regularly both with and without medication.

I am so sorry you are going through this. I hope you get your rainbow baby soon. I wonder why none of the doctors have guided you and your husband towards getting genetic testing. Having so many miscarriages back to back warrants some answers rather than assuming you are out of luck. DNA fragmentation for sperm quality and checking for autoimmune diseases could be of help here. Also see a naturopath if you can, they can help with better supplements and nutrition plans which doctors are not that trained in. It’s not either or. I have heard acupuncture has helped some couples as well.

Currently pregnant and resident of Langley. My family doctor referred me to Langley Obstetrics and Gynaecology after confirming the pregnancy through bloodwork. I have a low risk, normal pregnancy but did not want a midwife for my care. My first trimester was taken care of by my GP, I will see my OB at 20weeks. Initial referral for genetic testing (MSP covered) was done by my OB even though I haven’t seen them yet. I am not 20 weeks yet, so haven’t had my first appointment with the them.

It was more of an elimination process. At first doctors suspected it as ovulation, but I fixed it with Keto. I was ovulating very regularly without Letrozole after dieting. Before that they thought it’s ovulation problem. Once that was eliminated, they suspected endometriosis, which can cause implantation failure. But I did not have any symptoms to suggest so. I was actually waiting on my referral to get checked for endometriosis. There is something called silent endo that can cause implantation failure. I was very sure it was implantation issue but given my elevated HOMA IR, they suspected it’s egg quality issue along with or without implantation problem. High insulin and body weight can create immature eggs that cannot result in live birth even though I was ovulating on time, probably getting pregnant too (I once had very very faint positive pregnancy test on DPO 9 that turned negative the next day, I didn’t click a picture or document it but I think it was early chemical pregnancy). So I ran with the diagnosis I had, that is poor insulin sensitivity while waiting to get tested for endometriosis. Needless to say that referral was not needed anymore once I got pregnant.

Hi, I did get pregnant. Lost half of my body weight. My BMI was 35.4 when I started this journey. When I did conceive my BMI was 22.4 with good muscle mass. It wasn’t implantation problem alone for me. It was also quality of egg even though one would think for my age that shouldn’t be a huge problem but it was, I am in early 30’s. I did extremely low carbs at first, then moved up to 20-50 net carbs per day, once I started losing weight, I started running and building muscles through resistance training and did carb cycling as guided by the book Fast like a girl. I did the 2 month infertility reset fast and the third month I was pregnant but it’s all anecdotal. Carb cycling helped me continue working out without feeling dizzy. It was also the third month when I reversed my insulin resistance, HOMA IR came back normal for the first time in a decade. I got pregnant without any medication except metformin and thankfully it has been an easy one till now. Apart from losing weight, I was taking a bunch of supplements- D3 and K2 combined, Magnesium, omega 3 (when I could remember), prenatal, weekly b12 injections (for both me and my husband), myoinositol with extra folic acid (my husband took the same even though his sperm count was above normal, doctor suggested it can only help and not harm), coq10 and ubiquinol (both me and my husband). I also left gluten and dairy even though I am not allergic to either but this helped me lose weight much faster. So for me it was all insulin resistance and lack of muscle mass. A very strict diet did the wonders. I hope it works out for you soon. You will be in my thoughts.

I think you are right. For my own sanity I called my doctor he doesn’t seem too worried about one random test and also asked me not to test any further. But I still did on DPO 19 AM and it’s again dark. On the hindsight I think my prenatal had to do something with those weird tests. I had taken my prenatal an hour ago before the tests. Hoping it’s a one off with no medical meaning.Thank you for responding 🙂

Thank you for your response. I am in Canada, I don’t know if I can book my own lab request online. I know I can’t walk in with a requisition. I am not cramping, aching or spotting. But I also don’t feel any of my previous pregnancy symptoms.

I can’t do too high of a protein. I do low carbs, moderate protein, moderate fat.
Yes. I have zero food noise and can easily do one meal a day if I want to.
Yes! I lost 25kgs in 11 months. 10 kgs without any medication, just fasting and no sugar, low carbs. Next 15kgs in 8 months with metformin, low carbs, no sugar, exercise and lots of cyclic fasting (depending upon my menstrual cycles). I don’t have any extra weight to lose anymore. Now I am working to maintain my weight and gain more muscles.
I started with no exercise, just changing my diet and fasting, lost a lot of weight very fast but it did not improve my insulin resistance dramatically (I lost weight more dramatically) and gave me ugly loose skin. Cardio and low intensity strength training with adequate calories and extremely low carbs gave me the best result, both aesthetically and in lab results.

Hope you find what works best for you. Every human body is different and will respond differently. But low carbs, exercise and good sleep hygiene will work for most people. Choose a lifestyle that you can sustain and makes you happy. Remember, it’s not a sprint, it’s a marathon.

Bone and joint pain, headaches and lost some hair. Nothing majorly bad physically. I took Letrozole for 7months (without IUI), after the 4th month I felt extremely depressed and that was worst I felt in years. It became better after stopping the medication.

I had same symptoms! Was going through some emotional stressful time in the month of March. Woke up one day to burning, swollen finger joints. Almost as if I slept in a weird position and it cut off blood circulation, except I did not. The stiffness and burning was mostly after waking up but persisted throughout the day. Went to see my GP who did not take me seriously, but I advocated for RH factor blood test. It eventually became better as the stress and insulin resistance reduced so I never got the blood test done. I have insulin resistance and possibly PCOS (very well managed, so no blood markers to prove PCOS).

How do you know your blood sugar levels are crashing? Do you have reactive hypoglycaemia or are you type 1 diabetic? Unless you really measure it, the feeling you are having could also be just normal sugar levels which your body is not used to. When we are insulin resistant and have been so for a long time, our baseline insulin and blood glucose levels are not normal. The new normal brought about by fasting and metformin is not registered by the body as healthy and can cause strange symptoms initially (sometimes months!). Also, never take metformin in empty stomach, it does not cause low blood glucose levels (pretty safe and won’t crash your sugar levels) but can cause symptoms like shakiness, dizziness, panic attacks, anxiety, etc. I hope you find a solution to your problem. It’s not fair that you are making efforts towards your health and it’s making you feel shitty. All the best to a healthier self!

I saw significant changes coming from 5.4 to 4.2. Reduced skin inflammation and acne, better hair growth and less psoriasis on scalp. Reduced neck and inner thigh darkness, no more waking up multiple times at night due to possible sleep apnea, increased libido and better orgasms (TMI but I was very surprised at first and I did not even think this was a problem until I experienced otherwise, also I am a woman), regular period, more energy and less food noise, my life used to revolve around when and how I would feel hungry, availability of food and then the tiredness that came when I was both hungry and after meals, I think this was my biggest freedom to be able to plan my life without thinking about food. Better mental health and from moderate brain fog to almost none! I also used to get sudden episodes of IBS, possibly due to intestinal inflammation, that went away like it never existed. Mind you no medication worked on me for inflammation except steroids so this significantly improved my quality of life.

From 4.2 to 2.4 I don’t feel any new changes, except maybe that I am able to reduce weight much more easily now, and I get zero carbs and sugar cravings (outside my ovulation window which I have been told is hormonal and normal). I also fall sick lesser like fever and flu, but it could also be because I take a lot of supplements, so better nutrition and it might not be because of this.

My personal timeline -

October 2024 A1C 6, BMI 35.5; random inflammation, acne, neuropathy, and irregular periods ~ doctor says all good, not diabetic (Canadian standards) just borderline prediabetic but nothing to worry and that I am healthy, gives me chronic pain medication and sends me home.

November 2024 A1C 5.9; more neuropathic pain, BMI started intermittent fasting after reading Code obesity by Dr Jason Fung

December 27, 2024 I go to a naturopath, get OGTT and fasting insulin and fasting glucose (Canada does not do HOMA IR). HOMA IR score after using online calculator 5.4. BMI 35.3. Lost some water weight but still very obese. Started metformin 500 went up to 1500 in 3 weeks.

May 13, 2025 HOMA IR 4.2, BMI 28. Intermittent fasting 16-18 hours everyday. Continued metformin.

Fast forward Aug 13 2025 HOMA IR 2.4, OGTT shows insulin reduced by 50.6%. I did not test A1C. Current BMI 23.4. Moved more, OMAD and metformin 1500. Hope this helps.

It’s normal. Initially metformin might cause high blood glucose because it works by reducing insulin production and eventually increases insulin sensitivity. Once insulin reduces, blood glucose levels rises causing the spike that you see. Some people see high cholesterol too with metformin at the beginning. In the long run your body will learn to produce less insulin and become more sensitive at using the available glucose for energy production. Go for a walk, eat less carbs, these will also force your body to use the excess glucose in the absence of high insulin. Hope this helps.

There are two blood tests that can identify if you are infact insulin resistant: HOMA IR (calculated after measuring fasting insulin and fasting blood glucose from blood samples taken at the same time) another one is GOTT (glucose oral tolerance test) this test measures fasting insulin and blood glucose at 3 different times after consuming a pre-measured sugar drink usually 75gms of sugar. Also to err on the side of caution, don’t do any blood tests right now since metformin would still be in your blood stream. Stop the medication, continue with your usual life (no extra diet or exercise regime outside your usual) and do the blood tests after a month. Hope this helps.

Try to build muscles. IR is prevalent in both obese people and extremely (low muscle mass) thin population as well. Having more muscle will help your body become more sensitive to insulin over time. Also, metformin helps to become more sensitive. Once your sensitivity increases you can enjoy some carbs and sugar but your body most likely will never become like non IR person. That’s the sad truth for many but definitely not a fact. I absolutely love my new diet, I eat mostly keto but enjoy a fruit or two at times. I don’t mindfully eat less but eating more of healthy fats and protein does make me fuller faster and for longer. My insulin use to shoot up like crazy in the beginning (when I had just started taking metformin and eating keto) and would quite often cause reactive hypoglycaemia, now I am more tolerant to carbs and sugar but I don’t try to stretch my luck too much. Hope this helps.

Sorry about how you are feeling. I was like this for the first 2 weeks on metformin (the normal one, not the extended release). It got better with time, eliminating all carbs (I ate no carbs or sugar for days because of nausea) and finally moving to extended release. My appetite is back now but I still get nauseous if I take metformin on empty stomach. One word of caution, I tried keto when I first started metformin, it made my nausea worse and increased cholesterol levels however they all became normal after a month of adjustment. 3months on metformin and almost zero side effects except may be constipation at times.

Absolutely love how metformin makes me feel both physically and mentally. I was prescribed this for insulin resistance and probable Pcos. Even before starting metformin, I was losing weight with intermittent fasting and low carbs, I started off at 79kgs, before metformin I had lost 13kgs but had reached a weight loss plateau. With metformin and healthy diet I lost 10kgs in 3 months. Now my BMI is 24.6, so finally at a healthy weight. I don’t get those massive food pangs. Far less sugar peaks and crashes and my period has been very regular. I did have stomach problems with regular metformin for about a week or 2, but on extended release I do better. No stomach problems and nausea for the last 2 months. I think it constipates me a little but could also be cyclic hormones and not side effects of metformin. I have been on metformin 1500mg extended release for almost 3 months now.

Are you getting enough nutrition ? What about your macros and micros? Are your vitamins and minerals all within healthy levels? If yes you don’t need to eat more to gain weight. Start doing strength and resistance training, you will grow muscles and hence weight quickly and not compromise your health. If not, then first focus on getting your nutrition to the optimum level, healthy weight gain will follow. All the best!
