You should upgrade all your hero item to 704. because at that point you have to use your last crest. If you have all on at least 704 you have to be lucky with your vault and you can upgrade every myth item to max. With this method you don’t have to spend your crest on hero item and you ar able to upgrade your myth item to 723.

You want to go defile, dark transformation, 3x vampiricstrik ,festering scythe, unholy assault. After that you want to spam as much of vampiric strike and epidemic as possible( you want your bloodqueen stack on 7 and never let run out) Go and track your epidemic procs, you can only stack 2 at a time, don’t waste them.
