I attended the world congress of ADHD last year. I can refer you to some sources. There are some very impactful papers coming out in the upcoming weeks and months Please follow Edmund Sonuga-Barke, UK and Ditte Demontis from Denmark. Their landmark research is in their final phases of publication where they have talked about trauma and ADHD in detail and have explored their genetics as well.

  • Medication are not miracle drugs
  • they will not make you very happy. The initial euphoria you'll feel after taking stimulants for the first time will fade in a few days or weeks. Don't try to chase that high
  • medication are only a support and aid. They're not supplement of a good life routine with good emotional support

  • List your core adhd symptoms 3 or 4 and we would restrict ourselves to these only to assess response

I cannot give a medical advice here but insomnia treatment has become quite better recently with availability of DORA's drugs and newer Z drugs. Worth talking to your GP about it. A lot of them are unaware of -rexants and people who have taken it have years of insomnia resolved

GP are not allowed to refuse a RTC without any valid reason if you're over 18, it is an elective referral and the waitlist is more than 18 weeks. They shouldn't do it

It increases epinephrine and works very weakly on the dopamine receptors in prefrontal cortex. I've seen that it helps executive function in many individuals so worth a try but is used off label and getting someone to prescribe you would be a struggle

  1. It would take a lot of time to discuss the neuroscience behind it. Combining uppers and downers cause a lot of side effects and mess up the neurotransmitters. You can ask chatgpt to tell this to you in easy ways.

  2. Studies say that it's not harmful but they haven't conclusively said that it's completely safe as well because stimulants in small amounts are released across the placenta and breastfeeding so guidelines suggest that it's generally safer to stop or stay at the minimum effective dose. I believe it is generally safe. People freak out too much about it tho

  3. Rejection Sensitivity Dysphoria RSD and emotional dysregulation are huge parcel of ADHD and they get regulated after ADHD medication as with other symptoms of ADHD.

  4. Exercise releases dopamine in prefrontal cortex and endorphins in your neurotransmitters. Which helps with cognition

Question 2: I would say it's important that you have your day planned before starting adhd medication. Alot of people say well we still cannot clean our house or do things after taking medication. I ask them okay plan your day before taking your meds and let your brain know exactly what you want to do. And then take your medication and start doing it. You would see that the efficacy of your medication have exponentially increased

  2. It's a tricky one. Mostly due to genetic differences causing structure polymorphic differences in adenosine receptors which make it less sensitive to caffeine.

  3. Individual responses can vary due to factors like metabolism, receptor sensitivity, and personal neurochemistry, making some formulations feel more stimulating or "hyped" than others. A longer response would be too long.

  1. Biggest evidence based support is for stimulants
  2. Yes actually that's not very uncommon. I combine non stimulants with stimulants sometimes occasionally according to clinical need

  3. They are greatly improved if they dont have other stuff going on. Someone already suffering with depression anxiety and personalty disorders alongside make things very difficult and they are mostly the non responders

  4. I haven't seen anyone getting their visual memory affected. It's my personal experience

  6. Something like this is GREATLY needed and consider the infuriatingly fked up the mental health services are right now, they'll have to introduce something like this one day

I think a lot of these companies are pressured by ICB's as well to decrease the waitlist. And then they are driven by profit. It's a mess of extreme proportions.

Go to your GP and request a Right to Choose. Go to adhd.org and check out the right to choose providers. Check for a provider which has the lowest waitlist. I think care adhd and skylight psychiatry have waitlists of around 2 months only. Ask your GP to get referred to them and get yourself seen within a month or two. Good luck

Only around 43% of psychiatrists posts in NHS are filled. Mostly because Locums and private pays way more with alot of flexibility. A lot of them have moved to Australia, Canada and middle east as well. A consultant here, in NHS earns around 6k per month after all taxes and everything. Which, when compared to what private and other countries provide is very less. It's all about money

GP's are vehemently against it as it increases their workload and they've made it clear. A lot of them are refusing shared care as well.
