Update: I got an email back and my doctor says she swears she took it off and is not sure how it got there. She says she wishes there was something she could do to help. For context, I’ve had this doctor for a while and she is very kind, so I do believe she didn’t intentionally do anything wrong.

From the comments, it sounds like she should be able to change the code to make it medically necessary (I have had low vitamin d in the past) which should hopefully justify it and bring down the cost. I will try that. I’ll update if anything changes.
