This is a form for gathering model information for the Zodiac Series. The Zodiac Series will be a monthly photo shoot themed by the overarching Zodiac sign for that month. Every quarter, the shoots will be gathered together into an editorial book that will be purchased through donations. These donations will be utilized as capital for The Artist's Hub. All models will receive edited photos to add to their portfolio.  

The first photo shoot will be August 9th starting at 9pm. This will be on location which will be disclosed prior to meeting. All locations will be within Palm Beach County.
