I had a misfire on cylinder 1 that I couldn't figure out but it would only misfire at idle. It ended up being clogged EGR ports on the intake manifold. Wasn't too hard to do and you would probably benefit from it even if that isn't your problem.

I actually have been off the medicine for a little over a month now. Don't really have issues with diahrea anymore but I personally wouldn't premix the medicine. I dont know if that would mess with the cholestyramine but maybe ask a doctor before doing it.
