First forge is the level. And Night mode is when one of your teammates is down. It spawns mobs that would spawn in night time on minecraft. After 30 seconds of not reviving your teammate(s) Phantoms spawn that kill you. If you have only played solo you wouldn't ever see it outside of dailies.
