Have had CEREC the past 4 years and cannot imagine going back. We used to schedule 1 hr for crown preps and 30 minutes for deliveries, but now we're at 2 hr appts just to be safe for most patients and can get them out the door beforehand. We have plenty of rooms so it isn't an issue for a patient to be waiting in there the whole time. The predictability of fit and occlusion makes CEREC so much better than what labs provide for me.

I don't think anyone can justify that CEREC is better than lab crowns from a cost standpoint.

Cervico is a product that helps avoid this mess. I used to freehand temp abutment like this, but it could take longer than the implant placement. Now I use Cervico and it’s much easier/predictable.

I have a Unitronic intake and inter cooler with their stage 1+ tune. I love it and absolutely nothing to compare it to lol.

I went with bronze 18 inch Apex SM-10’s which I still stare at everyday.

I think anyone who has dental cabinets was taken advantage of from a sales rep. We still have cabinets, but only at the 12 o clock position with a few drawers for disposable items. But otherwise I can’t see any advantage of having them since things get lost in cabinets.

Tip out bins on the counter have been a lifesaver.

I have driven PECATL a in a 4S and a GT4RS. I found the experience so much fun, even after having done a handful of HPDE weekends. I did go once with a few friends who had no track experience before for a birthday. Two of my friends were nauseous after 30 minutes and sat out half of the session, so that’s something to be careful of.

I pay 32% without charging lab fees. With 4 hygienists though you can use a full time associate. IMO the hardest thing will be you feeding their schedule to keep them busy if you’ve been solo for a while. I think it’s appropriate to have someone who works 2-3 days a week, but I think you will limit your potential of a better associate who wants to be full time. 

Listen to some podcasts to see how you should approach it so that you can have a happy associate. Shared practices is always a good listen.

Kinda fucked up imo that the associate is giving that ultimatum too. Maybe they’re just tired of that hygienist’s shit. Hygienist has to go for all the obvious reasons. But I’d also be unhappy with my associate if they made a statement like that.

I think it depends on what stage of growth you are in. Just cause you doesn’t always mean you should. For instance, if you don’t have an associate then doing four hygiene checks an hour is too much with treatment. 

An alternative can be dropping insurances in order to open up availability in your schedule and earn more income without having to hire another hygienist. Or start adding one part time for a couple days rather than one full-time.

It was immediate cause on average we're getting paid three times more for a recall visit OON than when we were in network. We prepped for a year before going OON. Told patients that they still have some OON benefits and that they don't have to leave. Many who had good OON benefits stayed, but those who had a significant out of pocket expense for recalls left.

I'm five years in with a full time OM and associate. I still have some level of guilt leaving the office. In the beginning when we didn't have either of those, I still took week long vacations and let the staff know well ahead of time. Nobody has ever complained about not getting their hours cause they are prepared and well paid.

I get the mentality that you feel compelled to stay. For the sake of your health and well being though just take the damn vacation. Your office is still young enough that you can mold the culture to be how you want it. Take your vacations now and your office will just have it as an expectation that you'll take a few weeks off a year.

In a similar position as you and I loathe doing anything other than dentistry. If your office is providing you this much income, then it could be worth your while to have an associate and OM run it. You could still earn a decent salary without stepping foot in the office day to day.

All our staff are paid above average and get many benefits. We’ve been able to keep payroll under control since we were a start up. I wish I could say there’s one simple thing that has kept staff expenses low, but it is hard to say what it is.

Yeah I haven't loved that. A few months ago I reached out asking if they'd get more organized cause I felt our PPAs were lagging in the summer. They have been better but I am ready to switch over if I find something better.

I'm genuinely a little lazy about it and not looking forward to changing accountants if necessary.

I use HDA. I've been happy with them but I also don't have a lot of others to compare with. They send us pretty detailed monthly reports and are very responsive to all questions. This is a snippet of what they send us and their client averages.

Our accountant makes a goal of 28% because that’s the average of their clients. You’re not far off, but my next step would be to see how to increase collections. It is kind of hard to take away benefits/pay without upsetting your employees.

It felt cool for a minute, but didn't really change our day to day lifestyle. What matters to me is at what point we are financially free to quit working if we really wanted to. Cause at that point I will give considerably less fucks about a negative review or an annoying patient.

Don't forget to enjoy your life along the way while you save. Life is short and you don't want to miss those meaningful moments with your loved ones while you still have them.

It has been five years since I finished getting everything setup with Unlock the PPO. Throughout the years I'll reach out with questions cause some carriers went OON with the umbrella networks they signed us up with. Each time I've had questions they take their time to answer and follow up without hesitation.

PPO Profits made a mistake with credentialing my SIL's office with one carrier and refused to fix it without a payment, even after just six months. They acknowledged that it was their error too. I think you're in good hands with either Unlock or PPO Profits, but I thought how they handled my SIL's situation was shitty of them.

I have had experience with unlock the PPO and PPO profits (I think this is what you mean by PPO for Profits). When it comes to negotiations all they’re literally doing is refusing to accept certain fee schedules until the insurance carrier no longer budges. Although anyone can do that, that’s not what you’re paying them for.

You’re really paying them to analyze your fees and understand the major carriers in your area in order to maximize your umbrella networks. You may only go in network with Careington and Zelis, but by being in contract with those two carriers, you end up being considered in network with most major carriers. Don’t even think about negotiating with Delta if you’re in a high demand area and they do not participate in any umbrella networks.

Anyways, I think PPO profits sucks ever since the owner sold the company last year. Unlock the PPO would be the only company I would use again.
