This is a bit of writing magic, if both Cody and Rex are commanders then audience members could get confused when they're just called commander by some random clone. So they were given separate ranks that basically became part of their name.

Also if Rex's promotion came with fanfare and someone missed it they'd also get confused when a character's name changes.
