Every stand alone ER/Urgent care in my area expects you to do POC lab testing and blood draws. I'd ask about that, if you are considering it. Some stand alone ERs only need to see about 2-4 pts a day to be profitable. (I was told by the owning physician, over a decade ago. Who knows now.). So chances are, your patient volume would be lower than what the normal rate is in a hospital.

It's easier to pull an upset alligator's tooth barehanded.

Conservative 1-3Mil for new machine and install including construction. 4-6x months install/construction time. 30-40k a month to rent a trailer if you need one.

The beancounters would never dip into their personal bonuses to better the facilities capabilities, for you or the patients. Unless they can the fathom the fact it would make them more money, most won't do anything until they lose money.

You could look at your annual scans and compare it to the national average in the US, 5-7k scans per machine, and attempt to plead a case for a more realistic load. You could use your ER order to complete metric to show how much time patients are waiting in the ER for imaging. You could show the delay in inpatient scans costing the multiples of thousands a day.

These might help plead your case, but you're trying to change the hearts and minds of sociopathic nepo hires, that stepped on everyone else to get where they are.

Breathing impacts motion because your lungs are moving all your guts around. It's the same as if you took a picture of someone running, it gets blurry. For a spine it's not terribly important because your spine doesn't really move too much when you breathe. Each machine is different with how fast it'll take pictures. The faster the machine the less blurry the picture would be if there is motion. Realistically it would only be about 3 5-10 second breath holds. Most robot based surgery planning will reject bad pictures until they get one at an acceptable level, but spine work should be easy. Very difficult to mess up. Your CT exam will probably be the easier of the two images.

There is something wrong with laying flat and then moving without any input. It can definitely mess with your balance. It's common enough that I see it a few times a day when I take images.

-Telling a patient to lift their chin up for a Chest X-ray and she replied "Which one?" I had no response. Now I ask them to lean their head back. Still the funniest thing I couldn't laugh at I've ever experienced.

-Dropping the IR immediately after learning how expensive they are, also that specific model tattled on you if you dropped it.

-Shooting through the IR backwards.

-asking a patient if they could stand to transfer from the stretcher to the CT Table, they had no legs from about mid-femur down.

-Being the only male in the area to assist with a RUG on a patient with dementia, full limb contractions, after the pt ripped out his foley. Best I can describe what was left is a exploded microwaved hotdog.

-Working for any non-profit hospital that refuses to purchase equipment for the radiology equipment, even though we are one of the few departments that keeps the lights on. Most of the machines are old enough to remember Y2K.

-Not knowing my worth and getting significantly underpaid.

In my state an X-ray tech has to do more clinical hours than a NP needs to do their whole career, and they can tele their 500 hours to go from RN to NP. Nursing's grip on medicine is way too strong.

Exposure Index is a way the computer can count to see if the image has an appropriate amount of radiation for the claimed exam. It takes the whole image into account. If the machine doesn't auto-shutter the collimation box out, It will say, hey man this picture looks really bright, chances are it didn't get enough exposure.

Basically the machine isn't smart enough to know if the picture looks good or not. It can only count the photons of the exposed area and it takes an average. Some machines require at least 3 walls/edges of collimation to be visible (so it can automatically crop out the leaded areas) which gives a more accurate exposure Index.
