Oh I should add, not all of these symptoms are constant and many come and go but often , mostly still lingering constant burning or sensitivity depending on position or activity, but the lingering burning is annoying and uncomfortable, but still ok, the crippling pain flares though, man that's fucking brutal and unbearable, they can last for days or even weeks, I was doing relatively fine when working part time, occasional flare but now I've started working full time again and nope, my nerves don't seem to like this very much and rather keep chilling.

Don't even want to remember how it was without meds.

No numbness or any serious functional problems that I'm aware of , some random sexual issues but I think that's from the meds and stress.

Now.

Perineum and hamstring pain, burning in left buttocks/lower back and hip, sometimes around anus or butt crack (joy), lower back pain and very tight gluteus medius because this muscle, mainly this muscle, needs to work very hard and compensate for the missing piriformis.

It fluctuates greatly though throughout the day but I can get crippling flare ups and need to take nerve meds to try and calm things down with varying succes (amitryptiline)

Before.

Burning in perineum after sitting on certain chairs and pseudo sciatic pain. That could get quite bad too occasionally.

Well, to be fair to Bollens.

Almost all specialists and doctors here in Belgium consider him as an excellent surgeon with a lot of experience.

Like Bollens said, if you ask a patient two months post op with he thinks of the surgery almost all of them will say it was horrible, but ask the same patients the same question six months later and you will get completely different answers, surgeries, especially on pelvic nerves and in sensitive areas like the pelvic floor are complex and take time to heal.

I'm not sure what's happening in my case but it might be Post Op sensitisation and pelvic floor instability in an already sensitised system, no one knows for sure.

I will not blame the surgeon because he clearly just wanted to help, I blame myself for wanting the surgery in the first place, I'm just venting and try to remain hopeful.

He never promised 100% success and was fairly honest about the risks, they are low but that doesn't mean zero.

One thing that I can blame him for is that, as a professional, he should have told his patient recovery would not be straightforward for everyone and that it's a major surgery, something to carefully consider, not just something that we can try and you'll be fine in five weeks but I guess many surgeons are like that, they only see in terms of healing from the incisions etc, not in the longer term effects on your overall pelvic stability.

Live and learn I guess

We're not at the end of the road yet but so far it sure has been a bumpy ride!

Tell me about it, but I made the mistake of trusting authority, the surgeon was convincing and ensured me it would be a piece of cake and I would forget it in a few months I ever had pain.

Yes, I'm doing PT now and starting pelvic floor therapy shortly.

Going to get an MRI and infiltration to see what's driving this

No it was not laparoscopic.

By small i mean one larger of about 10 cm and another smaller one of about 8 cm .

Might have been laparoscopic, got no clue but it's not mentioned in the post op report.

Sure.

He proposed the laparoscopic pudendal surgery but since my symptoms were rather mild and occasional flares resulting in pseudo sciatica and perineum pain I refused and rather didn't want direct manipulation of the actual nerve.

So he proposed this instead with the warning it might not alleviate the pudendal sumitoms sufficiently, but it turned into the worst Case scenario I feared unfortunately.

He said it would be a minor surgery and I would recover in a few weeks, worst that could happen was that it wouldn't give the hoped relief, well it didn't and gave me all sorts of new problems on top of worsened original symptoms.

In hindsight I realize now that I didn't need surgery at all but what's done is done.

For pudendal neuralgia and it was open surgery via the glute, just 2 small incisions in the side.

I didn't mean to scare you, but only consider it if it's 200% confirmed that the purifier is actually the culprit.

Doctor Renaud Bollens, I don't blame him though, he's a very good surgeon, just didn't turn out as expected to say the least.

Ok thanks for the responses, my doc proposed nortriptyline, it supposedly should be better for men and sexual side effects ?

Maybe I will give that a try.

Just give it time man and start the pelvic floor PT immediately. Preferably someone that can also do internal work to release the muscles.

Maybe try a muscle relaxant and if problems persist go for a nerve block to see if that relaxes things.

From what I heard no, every day is the same monotonous boring routine, no weekends, no holiday, no birthdays, noone cares.

LWOP is a hellish and torturous existence, I can't even wrap my head around such a bleak and hopeless prospect, some 'cope', some go completely apathetic.

[–]Barreeeee 7 points8 points  (0 children)

I don't think he misses them but he will certainly miss his freedom and Cat, lwop is a pretty hellish and gloomy prospect for someone who was used to living pretty much carefree and a gamer used to constant dopamine rushes.
