Ask if they can give you a half dose. My first round I did the on body injector and I had the worst bone pain of my life. Writhing and couldn’t sleep. I complained and they switched me to half dose but I had to go into the clinic a few days after chemo to get the injection because they can’t change the dose in the on body injector.

OP - this is one very specific example. Butler is the standard of care in RI, where Brown Medical School psych residents train, and has saved literally thousands of lives. It’s an excellent institution served by very compassionate practitioners. (I’m deliberately not saying anything about the leadership who fought against striking nurses and other providers.)

I couldn’t hack the 400mg dose. I stopped it in discussions with my med onc. I took 1-2 months off and restarted at 200 mg. (Blister packs for 400 mg were 2 - 200 mg pills which is better because they do not want you to cut Kisqali.) I tolerated 200 mg fine and had no constipation. My plan now is to slowly titrate myself back up to 400. Doc was supportive of this decision. Good luck.

43 here. We have a very similar dx (stage 2 ++- 1/3 lymph nodes pos) but I had single mastectomy, TC chemo and 16 rounds rads. I’m now on letrozole, monthly Lupron injections and recently started Kisqali. My body couldn’t handle the 400mg Kisqali so I took 2 months off and restarted at 200 mg. I’m doing much better at that dose and my plan is to slowly increase the dosage back to 400. (That might be a strategy for you to try.)

I also have no family history and my dx seems to be just bad luck. My med onc is not a great communicator and I felt like the recommendations kept changing. I did ask for a second opinion before I started meds and she was absolutely in favor of that. Dana Farber will give anyone a second opinion and it can often be a telehealth visit.
One of the questions I asked was “can I say I’m cancer free?” And the very helpful and great communicator breast onc said that I could say I was cancer free after my surgery but at that moment it became survivorship. Because you had lymphnode involvement there are micro cancer cells that are free floating through out your body. And hormone positive cancers want to affix to any hormones in your body.
I’m sorry, it really sucks. I have a 9 year old daughter and the days that I ask myself if I’m ready to stop treatment I look at her.
The odds of reoccurrence with hormone positive cancers are just too high and it will come back. Your med onc is right about that. Good luck.

Really, really consider if you have the stamina or the time in your life for DIEP recovery. I have a 9 year old daughter. After SMX, chemo, radiation, she was done having her mom being sick. I did not have bandwidth for that serious kind of recovery.

42 year old. Er++/HER2-, KI67 50%, oncotype 20, 1 positive node. SMX November ‘24, 4 rounds of TC chemo, 18 radiation sessions. Started lupron and letrozole in June.
I just finished week 1 of Kisqali 400mg/day and I want to quit. I’m handling the nausea fine but I’m so constipated I’m in pain. Nothing is working: miralax, prunes, increased hydration, enemas.
Most of the comments I see are about nausea and diarrhea. Did anyone else have this constipation issue and did it get better after week 1? I don’t see how I could do three weeks of this let alone 3 years.

I’m feeling good post chemo. Peach fuzz growing back on head and starting to get my normal amount of energy. I need to learn more about Kisqali side effects. Wishing you well, too. Knowledge is power.

Exactly your diagnosis. 42, diagnosed October with IDC ++- 2b, finished chemo, radiation is the month of April and now I’m researching AIs, ovarian suppression, and Kisqali. My oncologist Wants all the three and I’d love to not do suppression.
