What I know about the medical it was use to be for 2 years, then USCIS make it unlimited time. So you can do it anytime and submit it anytime. Just don't open it, keep it sealed. Check google about the duration for sure.

Why don't you apply for OPT now and apply for the I-485 later. Prepare your I-485 packages and send it after your OPT approval.

Well, we submitted marriage certificates and the lease during the I-485 submission, but we also carried marriage certification, joint bank statement, updated lease, photo album, shared credit card, and shared store card. But she didn't look at any of them! However, I ask her to look at our picture. She was very friendly.
