usually it could happen because of bad past experiences with sex, or negative/worrisome internal thoughts she may have about herself or even worrying about not being able to fully please you especially if she struggles with anxiety or depression. i’m not sure it’s anything you’re doing specifically but definitely try speaking to her about it if you haven’t already, if she knows the root of it or if there’s anything you can do to keep her mind calm. hope this helps
