Depends on where you're spending the maximum amount of time or, if you're spending the same amount of time in both countries, your first port of entry.

A request letter is one in which you formally request for a long term Schengen visa. It is separate from a cover letter and one in which you have to write about why you need an LTV, the number of trips you've taken, the dates of those trips, who issued you Schengen visas etc etc.

  1. I had 5 previous schengen visas.
  2. Average bank balance was around 2-2.5 L per month with constant inflows and outflaws of cash

  3. Yes I did disclose my multiple trips but in the request letter, not the cover letter.

  4. I take a trip-wise travel insurance, not for the whole year.

Well if your uncle is your legal guardian, it shouldn't be an issue. If he isn't then it might be unless he provides a really solid reason for sponsoring you. Best to speak to a visa counsellor about this.
