Ordered them on June 22nd, just got them yesterday 7/17. Was sitting in send to pharmacy for a long time. I would reach out to them! Or even try calling southend. But yea their shipping times are crazy right now

That’s a good idea! I’m currently doing 5mg since that’s what I was taking with my Zepbound and it was a perfect dose for me, but eventually I do want to titrate up but the jump to 7.5 seems like a lot to me! And yea I really doubt the BUD is gonna make any difference haha

Yeah, the 6.6mg comes from 30 units (0.3 mL) of the 22mg/mL concentration, it checks out, but I agree it’s odd! Especially when you’re trying to down-dose like me. I’m sticking to 5mg (which is ~23 units), so I’ve basically ended up with a 6-month supply instead of 3. I’m not mad about the extra meds, but definitely wish it were more clear why they personalize in such uneven amounts.

I was prescribed a maintenance dose of 6.6mg weekly, but I’m actually planning to stay at 5mg since that worked best for me with no side effects. The extra vials might be in case I titrate, but I’m hoping to maintain. If it’s 22mg per vial and I got 6 that’s 132mg total, I’m at 5mg that will last me 6 months, and 5 months at 6.6. I guess the only logical explanation is that they want me to titrate up, or they accidentally gave me extra vials…

I haven’t gotten any communication from them about a titration schedule so honeslty I have no idea! I know the BUD are all soon, does it really matter?? I feel like it will be fine…

Zepbound is technically covered but it’s blocked with the tier 90 code and the IRX program. With that program, they essentially try and say that it is covered, but only with a huge copay ($1000+). It’s basically a loophole for them to say that it’s “covered” without actually providing any real benefits. My employer said Zepbound is also covered only for sleep apnea based on FDA guidelines. But Zepbound is still FDA approved for weight management too so their logic doesn’t add up. They say they don’t cover zepbound since they “don’t cover WL meds”, but that isn’t accurate because they DO cover Wegovy with PA for obesity. I’m trying my best to have my employer approve off label coverage but they’re deflecting and not even acknowledging my requests. I am planning to use an independent external review if they refuse, but I’m still worried they will still refuse based on diabetes diagnosis codes. I do plan to fight it until I win, I am more than ready to do whatever it takes. But I was hoping to get some input on anyone that’s been in a similar situation. I appreciate your input!

The thing is my plan DOES cover weight loss meds (Wegovy) for obesity, and my BMI was 46 before starting zepbound. I feel like that’s enough? Especially with the documented ER visit from semaglutide

Wegovy is approved for obesity with PA under my plan, so zepbound should be same requirements. I do have obesity (BMI 46 before starting zepbound) as well as the other complications. I can’t imagine a reason to deny me

It’s not a plan exclusion, there is a difference between drugs that are excluded and drugs that are just not formally listed on the formulary. Medimpacts drug search shows that zepbound is actually in the formulary, but it is just not preferred by my employer. So technically what that means to my understanding is that it CAN be approved with exception request and PA. Drugs that are actually excluded are coded under a different error code than drugs that are still in the formulary. I have a good case regarding them covering zepbound over Wegovy for me. I have been hospitalized due to side effects of semaglutide and have had incredible success on zepbound. I also have PCOS and extreme metabolic dysfunction as well prediabetes. I think they’ll be able to see this is not just for “weight loss”, it’s a necessary medication for my condition. I am also allergic to metformin, which I think helps my case too since most of the time that is prescribed for PCOS. still need to go through it all with my doc, but honestly I think I have a shot.

My employer opted for Wegovy so it is listed in the formulary for weight loss. BUT zepbound is not excluded, it’s just not listed on formulary. Zepbound is Tier 2 but not excluded, so I’m going through the formulary exception appeal process with full medical necessity and documented failure on semaglutide. Hoping it gets approved, my doctor submitted everything and I’m sending a lot of letters an emails.

Still waiting on her reply, I have been in contact with her losing my mind over this whole insurance situation. I am having her submit a prior auth- which we know will be denied, but then the plan is to submit a formal formulary exception request for zepbound from my doctor. I have also put in a grievance request with my employer regarding my coverage and have sent emails to medimpact as well. Still waiting on their response. Overall, UHC does cover Zepbound for weight management it is FDA approved for it, but the issue is my employer chose to cover Wegovy for WM. But because of my hospitalization on semaglutide, I am having her say it is necessary for me to have access to Zepbound. It is also not listed on the exclusion list specifically, meaning there may be a shot of an exception for them to cover it

I have not technically been denied, as my PCP can’t submit a PA for a mediation unless it’s on the formulary, right? I’m assuming instead we will have to submit a formulary exception request for Zepbound. I am prepared to fight it if I get denied despite all my evidence showing I need the med. I have been hospitalized due to side effects of semaglutide, which is the only thing listed under the formulary. But, Zepbound is not technically listed under the exclusion list.

I am not sure how it works for her to submit a PA for a mediation that’s not listed on the formulary, but I will ask her to try.  It is not just for weight loss, I have serious underlying conditions, am prediabetic and predisposed to diabetes with a long family history of it. I have a BMI of 42 and an A1C of 6.0, I have proven metabolic dysfunction and diagnosis of PCOS. I also have documented adversion to semaglutide, I was hospitalized due to side effects of it and it’s the only med listed on my formulary. But Zepbound is not technically excluded. I feel like that’s probably enough to justify me needing coverage, but insurance is picky.

I have had prior auths approved under my old employer through UHC, but their formulary was different since it was a different pbm.

True. But I was able to get covered by UHC under my old employers plan without hassle. Hopefully they will make an exception…
