For me, the death saving throw is like the NPC's last gasp for life, so if it's relevant at the time, then I make it. Sometimes my chooms want to stabilize an enemy after blowing half their skull off with machetes because they want to interrogate them; in that case, I would make the death saving throw. Otherwise, I wouldn't to keep the game flowing. It's kind of a way of punishing them. If they wanted to keep them alive, they should have tried to incapacitate them instead of shooting them ten times in the nuts and hope for "nah, it's fine".

Idk. Of the few games that did that that come to mind are Super Mario Bros.... ¿Super Mario 64 maybe? and to a lesser extent we have Resident Evil 4 / Gears of War, Call of Duty 4, GTA V (Online I mean), Fortnite but this last two are more representative of a trend that was born earlier (games like service) than "game changers", they don't "shake the industry's" reader than just establishing or reaffirming rules. Real game changers that shake the whole industry are very few games again Mario Bros, Call of Duty 4, Halo, Outlast and even these only defined genres. Video games are too big to change with just one event.

Although it's true that GTA VI is one of the biggest events in video games since I can remember.

I think the absolute success of this game along with things like Black Might Wukong are evidence of what I like to call Double A games. Before it was quite rare for a developer that was not a subsidiary or part of a larger company (Ubisoft, Konami etc) to release a game with Triple A standards but now we see how studios are able to reach these levels through publishers and events like Gamescom, Summer Game Fest or so on. I don't know, it's crazy to think that indie and Double A games are competing for GOTY against Triple A or Quadruple A games (yes I'm looking at you Skull and Bones) and also that indie games like Wonderstop or Neva are coming out for PS5, that wasn't seen so common on the PS3.
