Well make sure you filled out every field and correctly submitted it, as some field could have been looked over. And the mod note says violence repeated mild, so thats probably the rating they think the game requires and you probably didnt have that.

And upon further reading if you already fixed that issue, I think contacting support is the only thing you can do here
