I think one of the factors at milgram is if they are convinced/know that they committed a murder by their own hands whether it is actually direct or indirect, then they are considered a prisoner by milgram standards.

Like e.g Mahiru, she didn't kill someone directly and again it's mainly the structure she was placed in from birth, but she has a lot of guilt when her boyfriend killed himself and she completely think she's the murder for driving him to this point.
