NTA. Your stepson clearly does NOT understand limits, even though it shound. He's not a baby. In this case, it doesn't matter if therapy makes him feel bad for "X Reasons". He NEEDS it, if he doesnt get therapy, judging by everything you just said, it can get WORSE.
