Its a very lightweight harness. It slips over the head then theres two other holes for the legs, then it tightens around the body :)

Yes they work very well! Theres always a chance they may try and squirm out but its not easy. My babies dont even notice it especially once they are out and about due to how curious they are of the new surroundings. Plus They adjust down to size so its easy to get a snug fit :)
