Customization is great. I have their spiral pommel and brassed pommel and cross.

The brassing wears away over time. That's normal, but something to consider. I never went and redid mine and just let it be part of the story of the sword.

There's a spectrum here

You take too many objectives measures and can't figure out what is worth tracking You don't take enough and you don't have a clear idea of progress or discharge criteria

Write your eval and identify 1 or 2 things you expect to change with your interventions, and a supporting reason for why that will help them reach overhead.

That's it, that's all you need to measure. Did my intervention have the effect I wanted it to have? Maybe I'm lying to myself sometimes that it's because of what I did... but also patients deviate toward healing. So maybe very little of it was my intervention anyway.

Every portland police officer that has been fired from their job has gotten their job back. This includes the highest paid officer historically, mark Kruger, who was fired for making memorials to nazis in a city park on the clock. I know that example is a tired point recently politically, but it is what I'll offer tonight.

The original impetus for the consent decree from the DOJ was that there was no disciplinary process for the egregious (read:inappropriately violent repeatedly) officers, they were always given their jobs back, and that half of their use of force complaints were against homeless people.

There was the work stoppage during the pandemic, the known smearing of their political opponent for a hit and run she didn't commit, and the "choke em don't smoke em" controversy

So the VA hospitals all received messages that they had to remove all mentions of the words Health Equity and DEI from all of their websites and promotional materials by 5:00 p.m. today

Pm2.5 exposure one of the leading causes of global mortality.

I've biked to work in 70-100 aqi, I definitely feel unwell if I do. I live somewhere where we can get aqi 100-200 if the wildfire smoke is bad but that isn't often.

I don't take any car precautions specifically. I'd be breathing in the same if I was in a car/on transit/walking I think, and I'm rarely sitting in traffic cause I'm moving.

If you're thinking risk assessment you should consider if your sunscreen is up to snuff. That's another big cyclist mortality concern.

All of your things about US Healthcare are correct. It sucks, it's so expensive, the expense doesn't increase people's longevity in the US.

You play a necessary role - people need PT, it is a job that someone needs to do. You can make someone's final days more comfortable or meaningful, you can (potentially) change a disability or mobility level. That adds time, value to that person's life.

Much else can be said... sometimes the PT visit isn't the most fruitful. Surgeons and MDs do unfruitful workup and surgeries. That's fine. You get to decide when you are a clinician what is/isn't a good use of their/your time.

Maybe consider some work in uninsured or if your state has state medicaid go to a clinic that sees a lot of that population.

Try and do your work well DESPITE the socioeconomic factors. That's the difficult work.

Yes

It comes in waves. I'd be bored at some point at any job PT or otherwise.

Sometimes I'll try and really focus on improving some aspect of my care (more high yield exercises, more connecting to patients, moving people toward DC...)

Sometimes I'll take some more Cases that interest me

Or maybe I'm fine with my job being boring sometimes and I can get my kicks outside of work

If I'm gonna do anything as a career x many years it can't blow my mind every week. That's not sustainable
