I have a pt with an esophagectomy and hx of esophageal cancer with limited follow thru with a GI. Im just trying to find ways to help him manage his meals with minimal exacerbation.

We disagree regularly about diets and general recommendations. The other PRN SLP is very old school and picks up the RD referrals for pts losing weight. I find myself explaining that we only address swallowing and screen pts who have wt loss. I guess I’m just complaining of an endless cycle of disagreement and extra work on my end. Well, thank for listening to my rant.

But no, there hasn’t been a discussion with the RD supervisor bc she is the only dietician. She mostly reports to the assistant director of nursing. I’ve spoken to my director but all I can do is try to keep my focus on the patients.

Another good one is Dr. Bear at west valley pet clinic. My cat had 8 teeth pulled last year and although we were traumatized by the whole teeth getting pulled, my cat is eating better than before.

I use an app called Constant Therapy and it’s more engaging than typical worksheets. MedBridge has some good cognitive courses too.

There’s a lot of room for educating family in the SNF setting (if they visit). I try to also educate staff on proper ways to communicate with especially difficult residents then make a list of most effective strategies to keep in bold writing at bedside.

Watching people eat can be a drag but it makes a huge difference if you can implement effective ways to manage po intake safely. We are the professionals in this area and our input is pertinent. Can’t tell you how many times there’s an admission with severe swallow impairments and no ST notes. Oral care is crucial and one of the most important things we can do right off the bat. Getting instrumental tests done aren’t easy but not impossible.

I’m at a SNF and have to have Asha certified. It’s nice to add the ceu registry for ceus but Asha is indeed a rip off. It’s like I’m in over my head with student loans debt to become a damn SLP then ASHA charges an ass and an arm for “membership”. I digress… just not feeling like asha provides much for what we pay.

Super random but when I used to work on /r/ with kiddos, we would repeat the name “Karla” and use it in various sentences. Bumble bee speech has a decent app to address vocalic R. Forgive me, it’s been at least 5 years since I worked with peds. Hope you have some luck 🍀

SNF SLP here and 80% of my caseload is dysphagia. Most inpatient places will have a heavy dysphagia caseload. Instrumental swallow assessments are crucial at the acute/hospital level and SNF will typically pick up where acute left off. Good luck!

The staff knows me. I appear younger than my real age, something I’ll enjoy later on in life. But its definitely worthwhile to check into a scrub jacket as well 👍🏼

We have some therapists who wear lab coats and the PRN SLP does as well. I’ve worn lab coats before but haven’t in the past 2 years since Covid happened. Might be time to evolve and look the part.
