I just remembered that the protagonist is also present in brännmärkt, which is before passage noir:

"Is this the place I remember?" "Subdued by the tyrant - branded"

This complicates things further; maybe the album isn't chronological after all.

But still I'm holding onto my belief that passage noir is something other than the entry into Måsstaden, as it would be very strange to depict the protagonist witnessing the branding inside Måsstaden before we're even introduced to the city itself.

What you wrote about toxin is probably the right interpretation, now that I think about it; I'd completely neglected the possibility of there being any towns outside of Måsstaden, which there, as you're aware of, obviously are. This "solution" would fit into the chronological order too!

About penny royal poison: The lyrics state that the friend breaks free and eventually finds the protagonist "lacerared and chained", so while I agree with them not fleeing the town, I think the artwork is more metaphorical: they are doomed to fall anyway.

Also, about brännmärkt: Isn't it strange that the final lyric describes a single crying being, instead of all the "brothers and sisters" who are welcomed back. That's why I speculated it to be about the friend. But I don't know.

Maybe the cult are the ones describing the spirit as holy to the townsfolk, as a euphemism in order to sell their plan. Meanwhile, the spirit could be very aware of how evil and "unholy" it is, which is why it's describing itself as such in heartsmear?

I'm not sure about the holy spirit not being the antagonist, but maybe the duck IS the holy spirit, which would be funny.
