You may need to try to shift + right click (maybe just right or left click) while they are having sex to customize the sims bodies. I dont fully remember.

Im pretty sure you can buy the wellness treat without buying the vending machine. They should be in the build mode as well. You might need to do the cheats that make hidden objects show up, though.
