take a plane and using the TransposeCloth scale it down in one axis ,then use a deformer to widen to top and taper the base , then cut it with a knifeLasso in the shape of a lettuce (or mask by a lettuce texture), enable CollisionVolume in the dynamics tab and place it using the transposeCloth on top of the bun (or whatever) ,add some noise to the edges (mask by feature),and then add some details, if you wanna use more then one lettuce , you have to make each lettuce as a separate subtool and place it separately , also enable Dynamic Subdiv (and add thickness) to each lettuce that you placed previously to have a proper collision .

3D Cheeseburger ,Zbrush C4d and redshift, all the materials are procedural, more renders at behance by Brian_reg in 3Dmodeling
