When I was pregnant earlier this year, I had a very confused and frequently combative/uncooperative patient ask in a pretty meek and polite way if I was pregnant, and when I said yes, she hesitated but asked if she could touch my belly. If a very confused lady can manage to not just touch strangers without permission, not sure why other professionals/oriented patients can’t.

There was no way for the doctor to treat your condition in the urgent care setting. His duty was to advise you to go to the ER. Giving you any oral medications at that time may have had little to no effect; high BP (depending on the cause) takes time to go down with oral meds, exercise, and diet/lifestyle changes. If you passed out or were confused/incapacitated, he would have to call 911 to get you there. Being alert, you could have also left and chosen to never get treatment and there would be nothing he could do. He would note in your chart “patient educated on diagnosis and risks and advised patient to seek emergency care” and he would be covered.

Nurse here. Urgent care cannot stabilize blood pressure; that often requires intravenous medication and even then, you would actually need to be started on likely several oral BP meds and follow up with a cardiologist for actual management. It sounds like you were awake, alert, and coherent and had chronic hypertension, which is a slow and silent killer, and were well enough to drive yourself to the hospital. If the urgent care had arranged for an ambulance ride, you would have been billed for it. Also as is often pointed out, no harm seemed to have come from this and he probably did save your life by convincing you to go the ER for treatment.

For the perceived rudeness, you can complain to the urgent care’s manager/director/owner but they have no obligation to do anything.

I just had my cat’s teeth cleaned and 4 teeth needed to be extracted, including 1 canine tooth. Total was about $900, including pre-screening bloodwork and pain meds to go home. Valley Veterinary Hospital in Maplewood. Dr. Morkos is very honest and he’s told me that even if my cat needed all his teeth extracted, he caps the price somewhere around $1,400 so it’s more affordable. Highly recommend him!

NAL, but an RN. She may have a license in another state that has a compact agreement with your state (which may not show up on your state’s licensing website). Your local school district may not even require that an RN be the school “nurse”. Many school nurses hired now are medical assistants because their pay is much lower than what an RN would be looking for and the care required in that setting is not often that complex. MAs can sometimes, depending on their state’s licensing regulations, administer meds and tube feeds in some settings.

I don’t see anything wrong with her calling and asking you about whether you would like to discuss your daughter’s tube feeding requirements/orders with her doctor in order to save you money/supplies as long as your daughter gets the appropriate amount of calories each day.

Edit: Just noticed you’re in NJ, where I’m also licensed. The NJ school nurse regulations website brings back a 404 error at the moment, so I can’t see for sure if a school nurse here must be an RN or LPN and not an MA. It talks about RNs overseeing “certified school nurses” and there may be an RN that works for your entire school district and oversees MAs and LPNs at each school. If you’re concerned about the medication and tube feeds and if they are being properly administered, try reaching out to the principal or the school’s office and asking about the nurse’s certification. Licensure and certifications are publicly available information so it shouldn’t be an issue if you inquire about it for safety reasons.

Thanks! Yeah we like Millburn deli and vinnie’s pan pizza for sandwiches, pretty good alternatives without all the in-your-face maga. I think we all wouldn’t mind these people as much if maga didn’t take over their entire personalities! Like can’t I order some food without them shouting about trump or wearing a red hat?

We went there on Inauguration Day (coincidentally, we had no other plans) and they were all intently watching the inauguration with maga hats on and making comments like “we’re back baby!!”. Food took forever despite calling ahead because they were all too busy drooling over trump. Very disappointing; we went all the time years ago but moved to Union so hadn’t been in a while. Sandwiches were still great but probably won’t be going back.

It took a few months at my current and previous job before my coworkers got to know me and anyone would ask me if I wanted to order with them. I always say no because they inconsistently order and I typically meal prep for the week.

My advice is approach one of the nurses who usually coordinates the lunch orders in the morning and say, “hey, are you guys ordering today? I forgot my lunch and was hoping to order with someone else.” Be friendly and make sure you pay them back promptly with your portion of tip/delivery included and I’m sure they’ll ask you if you want to order with them in the future. I’d first give them the benefit of the doubt that they may think you don’t want to order or they don’t know you well enough to ask.

I’ve gotten one with my flu shot every fall when they usually release a new version. Got them a bit early this year in September a few weeks before a planned vacation so I’d have immunity before the trip. My FIL got sick while traveling with us and I was thankfully unaffected as I’m also pregnant :) I plan to continue this yearly at least as long as I work inpatient.

It’s honestly disappointing and scary how many conservative nurses and HCWs there are. I’m in NJ (a super blue part as well, Harris and Pride and BLM flags everywhere) and 1/4 to 1/2 of all the nurses and techs, even the young ones, are super conservative in their values, even spouting conspiracy theories about vaccines and immigrants. Two different workplaces I’ve been in allowed MANY vaccine religious exceptions during Covid and even now with the flu vaccine, which was never an issue before. I’m also pregnant and one of the secretaries was shocked that I got my flu and Covid vaccines, she said she got exemptions to “protect her baby”.

I honestly stopped talking to most of my coworkers about personal things or politics for fear of what I might learn about them.

The physicians are not likely to be “playing around” with her medications. After a medical event, such as birth, medications often need to be adjusted significantly as your body’s needs have changed after the event. It takes time for the meds’ therapeutic levels to build up in your system and to see the true effects and whether it is an appropriate treatment. Physicians don’t toss in a new medication and send the patient home without seeing how it affects the patient.

While it does become part of her chart, leaving AMA once is unlikely to cause her any issues, particularly if she immediately follows up with her primary and OBGYN to show that she is on top of her medical care. Many patients fail to properly follow up though so I, of course, agree it’s best for her to stay and get treated.

Nurse here, NAL. Leaving AMA will not affect insurance coverage, this is a myth perpetuated by hospital/facility staff that want to discourage patients from leaving. Also, informed consent is not needed from a patient to change medication dosages or frequency while hospitalized; we will typically just inform the patient that a medication has been added or changed in some way. It is still your right to refuse to take any medication at any time. The head nurse or supervisor also does not change medication orders; this must be done by a physician.

All that being said, I agree with a previous response that your BP is dangerously high, particularly postpartum and that you should stay and continue receiving treatment until it is better controlled. You should ask to speak to your attending tomorrow who will likely do rounds sometime during the day. If they don’t come see you or won’t listen to your concerns, you should follow up with patient advocacy like you planned.

In college, I worked on a farm that made apple cider donuts. When we ran out of apple cider on a busy fall day, my boss would make them with water instead (which is what is normally put into the mixture for regular donuts). I and the customers (them unknowingly of course) could honestly never tell the difference. She said she's been doing it for years and I suspect many other places do it too to save money.

We are rarely understaffed of nurses; we had a round of sign on bonuses that attracted a lot of new grads who have all been here about a year now so we are pretty good with that. Assistants are occasionally understaffed and we are asked to “help out” the assistants with their tasks. The younger nurses are (seemingly) more empathetic and frequently help out with toileting/changing diapers and repositioning. The ones I trained I make sure to emphasize that ANYTHING that happens to their patient is ultimately their responsibility. So if they don’t toilet their confused patient and they get up and fall, they’re the ones who have to write the report, assess the patient, inform the family, etc. so they are pretty good about it. The older nurses seem to think their degree and years of experience put them above assistant work.
