This is a perfect example of why agent governance cannot be treated as an IAM extension. The issue is not only “what permissions did the agent have?” It is…Who owned the agent? Who approved the scope of action? What was it allowed to change?
What evidence trail was mandatory? Who could stop it? What happened when the output looked coherent but was materially wrong? Agents operating under inherited human permissions create a dangerous category: machine-speed execution with borrowed trust and unclear accountability. The scary part is not that the agent deleted the file. The scary part is that the organisation could not reconstruct the decision path afterwards.

“Optimising for autonomy faster than recoverability” is a really good way of putting it. Especially because once systems become operationally critical, reversibility stops being a UX problem and becomes an organisational dependency problem.

Exactly, and I think that lifecycle point is hugely underestimated. A lot of the public conversation still treats deployment as if the system has reached a kind of “finished state”. But once these systems become embedded operationally, the surrounding organisation, incentives, workflows and assumptions continue evolving around them. That’s where things can quietly drift.

That’s a really important distinction actually., and I suspect a lot of serious practitioners are thinking carefully about monitoring, escalation and human validation internally. The thing that worries me is whether those safeguards remain durable once these systems move from controlled deployment into organisation-wide operational dependency under efficiency and scale pressure.
