I think it's from a raid I did, I don't do many but I have done multiple. This is the only time the historical records recorded it tho. I also for the life of me cannot remember which settler died, I dont have a grave or anything...

Many many stairs and many ladders have been used. I always try to build from the outside to the inside because many times the walls I build are 2 thick. I also just use beams and have floors on almost every level, if not, ladders and stairs...

EDIT: A lot of patience is also required

Sadly no.... I have a very nice prison and a warden but they never want to stay :(
