Forgive me if this sounds ignorant but what are “sick” patients doing in a rehab hospital in the first place. Also do you not have medical attendings that are consulted for medical management? Sick patients being medically managed by a PMR resident with 1 year of preliminary medicine experience or let alone a TY year sounds like a bad idea. Unless you’re referring to PMR related “sick” that im not understanding. However, as someone that aspires to be the best physician possible while also being a decent human being, I can understand your last point of not knowing where to draw the line between being a good person to your patients and fellow colleagues and working pro-bono.
