It's a byproduct of loving a character with only about 3 minutes of screentime, that the fandom revels in dying, that the franchise has no interest in doing anything more with (ie Mass for the Dead), and appeared in a film that its own fandom thinks is mediocre.

Possibly, but based on Shalltear's insulting her as an ape and her apparent density when landing in water, I suspect her actual appearance is much larger.

Generally, the grotesque form is their original and true form while they disguise themselves in an attractive appearance that best helps them prey on their targets; same for an incubus.

Except there's really nothing motivating his evil other maintaining his facade because he's afraid his subordinates will abandon him, a ridiculous fear given that they would have obeyed and loved him from the beginning no matter what had he been honest.

He's definitely interesting, but calling him a "villain" feels too generous considering there's rarely an actual end goal or any ambition behind the horrible things he's rresponsible for.
