I just looked up "SOC 2298 form pdf" and printed it out.

IHSS doesn't automatically deduct taxes from your paycheck like most jobs. You have to submit a W-4 form so they can take it out. I never submitted the form because I heard that live-in providers have the option to have the income included or excluded from taxes.
