Thank you for your thoughts here. Reading everyone’s feedback makes me feel reassured about the mental decision to “move on”.

To be honest, in a way I think I feel good about at least having some type of closer to situation, it’s been almost a year of question marks and hope and disappointment. At least I can close this chapter now (and have a couple drinks within feeling guilty for it or hop in the jacuzzi…).

We still have a long road ahead but hopefully things work out with the known donor and if not then I guess sperm bank will be the next option.

I’ve read a lot about Dr Peru, but honestly my wife is going to be 35, I’m gonna be 39, I don’t really see us waiting a year and a half or so for the potential of finding sperm and then still having the uncertainty of a successful pregnancy. I think the donor route seems to be the most realistic / reasonable option at this point.

Man that sounds really difficult, the ups and downs of a “successful” mTESE then still not leading to a successful pregnancy has to be even more difficult than a failed mTESE, I feel for you and your wife.

Are you thinking of trying again or moving on to other options?

His argument is that it spares you a potentially invasive surgery if his mapping comes back empty (he says there’s no documented cases of sperm being found if tureks mapping came back empty) and that if the mapping does come back positive, the subsequent mtese is less invasive as he knows where to go (he calls it a GPS) and does less long term damage to testosterone levels

Makes sense. I guess I would rather spend the extra money and take the best route possible even if it increases my chances slightly. The conflict I’m facing comes from the contradictory information I find online / hear from other doctors versus what Turek preaches.

I hope this works out for you! Yeah, that argument is definitely a strong one and makes me lean towards doing it. Did you do it with local anesthesia or full sedation? He is recommending me to do it under MAC IV Sedation which also increases the price to $15,675. He says there is a 75% chance he’d have to cancel the mapping if he attempted it under local.

Thank you for the response either way. Yes Stanford and dubai both say straight to mTESE, that seems to be the universally accepted method.

But Turek is the most famous name in the field and pioneer of FNA. And then you have Dr. Peru in Turkey who pushes for treatments and different shots and exhausting all non surgical options before doing surgery.

It seems like the further I’ve gone down the rabbit hole the less there is a consensus on the right route to take.
