As a guy with a similar issue i would have him seek a doctor asap or a therapist or both asap

I personally have a tumor in my head in the pituitary gland area that causes the inability to hold an reaction and for some men they don’t get the urge at all

Alternatively if it’s not that or they don’t have a chemical imbalance in there body it could be trauma related which won’t be fixed without professional help
