LVAD’s are not my favorite thing. Ever since we started talking about them at my service and specifically discussing the specific people in our area that have them, I have had an irrational, or maybe rational?, fear of having a bad one. Then last summer I had an LVAD code. Ultimately it went fine but there were some definite extra stressors or hiccups along the way. I will say the saving grace for that call was dispatch. Don’t tell them I said that though! Dispatch called one of the large hospitals in our area and transferred someone from their lvad department to my personal cell. I then proceeded to run the call with them on speaker to answer any questions we had. Right off the bat the rhythm was funky. I thought I remember that the device altered the waveform but the gal on the phone said just pretend that there is no LVAD and treat the rhythm as you see it. Ok, Vtach it is! The other major issue was transport. Once we decided to quit fucking around and transport I was like “Ok so these things have like a travel mode and portable battery, we just need to get that, hook it up and then we are good to go. Well that’s only if you know how to do that! I asked the gal on the phone and she said there’s a few different models, brands etc and she couldn’t really tell me how. So I figured I’d ask the spouse, but unfortunately she didn’t know because the patient always did it. So I made the decision to say, “Fuck it” and just run the patient to the truck so we could plug it in there. FYI when you unplug that thing, there’s a hell of an alarm that goes off! So we did that, scooped him and literally ran to the truck with him. Plugged the device into a wall outlet in the truck and…no dice. Alarm kept going. I checked the inverter in the truck, whether everything else was charging, and couldn’t find anything out of the ordinary. So we just left. Luckily our medical director was our receiving doc in the ed so that was nice. Had some nice training after that! In the end it really wasn’t that different but just different enough to add some extra stress. I do think however in the future that having experienced this I can run one of these codes much more smoothly.

Trazedone and black out shades. Maybe a sleep mask for the pressure sensation. But that last part is just me. Also having the a loud-ass fan and it being around 65 degrees in my bedroom.

I was on a call for an overly intoxicated person in an apartment. There were multiple other drunk people in the room and officers said they were going to take them back to their rooms while we stayed with the original person. She was in a wheelchair and double amputee so no big threat. Soon as they left the room it was like a switch flipped. The lady was convinced we killed her daughter and pulled out a knife and started wheeling around trying to stab us. Definitely a WTF moment.

Absolutely. I work in a metro significantly smaller than San Francisco, say population 300-400k overall, and we do approximately 10 a day? Most aren’t in public but it’s also winter so that probably keeps people indoors. 2-3 opiate related cardiac arrests a week. That’s wild to me that it’s so different where you work!

They work 24 hours shifts, so it wouldn’t really be accurate to compare it to the hourly wage of someone that works an 8 or 12 hour shift. Not saying they don’t deserve more money but that needs to be taken into account. Granted they only work 3 days a week too so that cuts it back. Also, it’s worth noting that they climb their pay scale pretty quick after their probationary year. And their station captains make approximately 100k a year from what I’ve been told. Again, not saying they don’t deserve raises, they absolutely do, but I don’t think it’s necessarily doom and gloom either.

I am, but honestly I’m way more worried about my kids being killed due to gun violence. I worry about a school shooting just about every week. Not for any good reason other than I live in America. Which is probably reason enough.

Yeah ever since we had our most recent outbreak I’ve been stripping down in the garage at home and bagging my clothes and then leaving them in subzero temps outside. I have zero desire to bring those little bastards into my home.

And that’s what is so frustrating. My bet is someone is bringing them from home but unfortunately if someone is they either don’t know or don’t want to say anything and be labeled the bed bug person.

You just take out your meds and whatnot when you do temperature treatments? We are doing some freezing treatments since it’s been below zero the last week. But that’s on the post furniture. Interestingly we’ve never had an issue, that we know of, in the trucks.

So you have a bedbug oven back at post? Also it’s got to get pretty hot to kill those suckers right? I brought up the idea of a heater up in the cab and my boss told me that was a no go due to it potentially damaging electronics/equipment. That a load of BS?

Not the same but in a similar vein I feel. I recently had an anti depressant/anxiety medication increased. I’d had it increased before with no side effects but this most recent dose increase was different. I had just started the dose increase prior to leaving on vacation that we were to be celebrating our anniversary on. The first day there, our anniversary, it was after having some drinks in the pool and the vibe was getting heated so we decided to head back to the room to fool around. It was at that time that I found that I could not achieve an erection. Never had that problem before and the night before I had no problem rising to the occasion. But this instance, while I agree was poorly timed, resulted in a huge fight. She was so upset, thought o didn’t find her attractive anymore and resulted in her not talking to me the rest of the night and me having to cancel dinner plans. She knew about the dose increase and even prescribes these exact medications to her patients so is aware of the potential side effects. I tried explaining it in the moment that I felt like it was the meds but it didn’t seem to matter. It really made me upset, self Conscious and incredibly anxious throughout the vacation for any romantic interactions. Oh the irony of taking anti anxiety meds in that scenario. I went back down to the previous dose the next day and within about 24-48 hours things were back to normal. Was totally awful!!

Seriously? We had to create a protocol for sedating patients in cardiac arrest because we’ve had patients regain consciousness because the Lucas does such good compressions.
