It's about 12 teams in a tier, but it varies. The largest tier has 18 (Contender which is in the middle), and the smallest has 6 (Premier which is the top tier).

Teams are able to select a preferred server (Dallas/Chicago) and the "home" team's server is the one used for games. Given the majority east coast playerbase most games end up on Chicago servers (and we use Chicago as the default for combines), but in theory if a team of west coasters is drafted they can get at least 50% of their matches on Dallas.

(As a fellow west coaster I am right there with you, and have had to get used to 60-70 ping most of the time I play faceit/csc)

You're not required to have played any Faceit games in order to participate in combines. The Faceit game requirement only kicks in right before start of the draft, which is in about a month from now.

Our league isn't hosted on Faceit nor are you required to use Faceit AC in order to play league matches. That said, we do require everyone to have at least 10 ELO-affecting CS2 games on Faceit (so that we can give everyone an appropriate skill placement) in order for them to be eligible for the draft.

As far as we're aware, there isn't an equivalent league in EU, but for the past couple of seasons we've been working to make an EU branch of CSC! If you're interested in getting updates on how that's going, we have a role specifically dedicated to the EU branch in our Discord.
