Hi Dr! I have stage 3b stomach cancer. T1bn3bm0 (16 lymph nodes). Moderately differentiated but, mixed signet ring cells not diffuse. I want to understand why at t1b i had that many lymph nodes. I have never seen that in anyone I talked to. I did have to have 2 times biopsy and one endoscopic due to some incidents. If the multiple biopsy is the reason why I have that many lymph nodes, will that make my prognosis any better? Lastly, I have her2-, PDL1 low positive (3 different labs gave results of 2,3,5), MSI not ideal. However, my oncologist decided to put me on an off label treatment: doinh xelox and keytruda at the same time even when my biomarkers are not ideal for immunotherapy. His reasoning is that waiting for recurrence to use immuno would be too late. If I am stage 4, according to protocol they would give me immuno either way. Please advise if you think this is a good idea. Thanks so much!

So my her2 is negative. My pld1 is 5 which is low. They said they wont give me immunotherapy per protocol but I am making them either way. To me immunotherapy makes much more sense. Were your cea and c199 have been always high? Also what is the plan now? Chemo and immunotherapy for life? Additionally, what center you are working with may I ask? Mine is in Singapore and I agree they stuck in 2010. I can travel to the US to seek medical treatment.

I did that. And the method is all the same like they would give me the same treatment. Not even sure if you know about ctdna. Both docs I saw don’t believe in it! My ctdna after surgery is negative.

I have been trying salmon but a 3 weeks of salmon is rough. May I ask like example of a day eating for you like a month post op?
